How it's calculated

The monthly payment uses the standard fixed-rate formula on the full amount borrowed. An origination fee is a percent of that amount and is taken from the cash you receive. You still repay the full amount plus interest. The effective APR is the rate that makes the same payment equal the cash you actually receive. With no fee, it matches the rate you entered. The schedule keeps full precision until amounts are shown to the cent.

Payment = P × r ÷ (1 − (1 + r)−n)
P = amount borrowed, r = rate ÷ 12, n = months

Questions

How is the monthly payment calculated?

The payment uses the standard fixed-rate formula on the full amount borrowed, the interest rate, and the number of months. At 0% interest, the payment is the amount borrowed divided by the months.

What does the origination fee change?

The fee is a percent of the amount borrowed and is taken from the cash you receive. You still repay the full amount plus interest. The effective APR is higher than the note rate because you received less cash.

What is included in the total cost?

Total cost is every scheduled payment: the amount borrowed plus interest. The fee is part of the gap between that total and the cash you received.
