How it's calculated
Both payments use the standard fixed-rate formula. The current payment uses the balance, the current rate, and the months left. The new payment uses the same balance, the new rate, and the new term. Fees stay outside the new balance. Monthly savings is the current payment minus the new payment. The break-even month is how many of those savings it takes to equal the fees. Interest saved is the interest still due on the current loan minus the interest on the new loan. Fees are subtracted from that difference. A longer term can cost more even when the monthly payment drops. At 0% interest, a payment is the balance divided by the months.
Payment = P × r ÷ (1 − (1 + r)−n)
Break-even months = fees ÷ monthly savings

Questions
How is the current payment calculated?
The current payment uses the balance, the current rate, and the months left. The new payment uses the same balance, the new rate, and the new term. Fees are not added to either balance.
How is the fee break-even month calculated?
Monthly savings is the current payment minus the new payment. The break-even month is how many of those savings it takes to equal the fees, rounded up to a whole month. If the new payment is not lower, the fees are not paid back by the monthly difference.
Can a lower payment still cost more?
Yes. A longer new term can charge more interest even when the monthly payment drops. Interest saved is the interest still due on the current loan minus interest on the new loan. Fees are then subtracted. A negative result means the refinance costs more.
